Covid Safety - Client INFO, Preparation & MANDATORY QUESTIONNAIRE
As discussed in our correspondence, you must complete a covid screening questionnaire and declaration sometime prior to your shoot, which should only take a few minutes. The questionnaire is located below. This webpage also contains some additional covid-related safety information for photoshoots.
The covid screening questionnaire is mandatory for the safety of everyone involved. If the questionnaire has not been completed by all participating individuals by 9AM on the date of your shoot, the photoshoot will be cancelled by the photographer. The questionnaire is below, or can also be filled out at this link if for any reason you are having trouble viewing the form on this page.
Please wash hands thoroughly immediately before the shoot. If possible, please have hand sanitizer handy.
While you will of course not be wearing a mask in your photographs, please arrive at the meeting spot with a mask on. During your shoot, please keep your mask handy (in your pocket or purse, if possible) for everyone in your party, in case for any reason a situation arises where the photographer may have to get closer than 6 feet to interact with you.
Covid Safety - Photographer Protocols To Keep You Safe
The photographer will wear a mask at all times, and have personal hand sanitizer handy.
The photographer will wash hands thoroughly between each shoot.
The photographer will maintain a minimum distance of 6 feet during the photoshoot.
Every photoshoot participant will be required to complete a screening questionnaire.
In the event of onset of symptoms, or exposure to a known or suspected case of covid-19, the photographer will cancel the shoot for the protection of all involved.
If it becomes evident that you or anyone in your party have symptoms, the photographer reserves the right to cancel your shoot on-location.
